CostReport.net is operated by Codexusa LLC, a Wyoming limited liability company. Codexusa LLC is the entity responsible for this website and the personal information described in this policy.
What we collect
When you submit the contact form, we collect what you enter: agency name, your name, email, phone, agency type, CCN (optional), fiscal year end, MAC, and your message. We also record the date and time, the page you submitted from, and your browser’s user-agent string to help prevent spam.
This site does not use advertising or analytics cookies. Our hosting provider (Cloudflare) may process standard technical data such as IP addresses to deliver and secure the site.
How we use it
- To respond to your inquiry and discuss our services
- To keep a record of business inquiries
- To protect the site against abuse
We do not sell or rent your information, and we don’t share it with third parties for their marketing.
Where it is stored
Form submissions are stored in a private Google Sheet that only CostReport.net can access. The service providers that host this site and store submissions (Cloudflare and Google) process data on our behalf.
Health information
Please do not submit protected health information (PHI) through this website. Records for engagements are exchanged through a secure method agreed with each client.
Retention and your choices
We keep inquiry information as long as needed to respond and for reasonable business records. To ask us to update or delete your information, send a request through our contact form.
Changes
We may update this policy. The effective date above shows when it last changed.
